[Y]our First Appointment
 
 
If you are coming to ResponsicareSM for the first time, we ask that you arrive 15-20 minutes before your scheduled appointment time in case we have any additional information we need to collect from you.

Please make sure that you bring a list of or containers of any medications you are currently taking, including the name and dosing information. We also ask that you bring in all of your insurance information so that we may verify it in our computer system.

Finally, we ask that you familiarize yourself with your medical history, including all past and current conditions that you may or may not be being treated for actively.  If you have current serious medical problems and are taking medicines that are helping to keep you alive and functional, don't you think you should have intimate knowledge of those conditions, your medicines, and the doses of those medicines you are taking at all times?  Makes sense, doesn't it?  Remember, our ability to treat you correctly is highly dependent on our patients and their families giving us ACCURATE and HONEST information. 

You can expect to spend at least 45 minutes in the office if you are here for a complete physical, less time if you are here for a minor problem, or, perhaps more time if your problem is urgent or very serious. This is essential so that we may ensure a complete medical history, accurate diagnosis and treatment plan.
